Nurse Associate

We have an exciting opportunity for a Nurse Associate to join our existing, expanding and dedicated nursing team consisting of a lead nurse, Nurses, Healthcare Assistants and Phlebotomists.
You will provide a continuation of health care incorporating health promotion and preventative work for the patient population. You will be enabled to consider gaps in service provision for which your training may have enabled clinical skills that can be transferred into primary care.
You will work as part of a multidisciplinary team, accountable for the delivery and standards of care provided to patients ensuring safe, effective, person-centred care.
Your role will be Central to the nursing team bridging that of the HCA and experienced GPN in the delivery of wound care, Promotion of health in long term health condition reviews, NHS and SMI health Care checks supporting our mixed demographic population of ~12,500 patients.
We are a forward thinking, dynamic and friendly Partnership., and work across three surgeries in Norwich We enjoy excellent working relationships with neighbouring practices, working collaboratively as part of the West Norwich Primary Care Network. We have been providing care to the people of Norwich for many years
We have three GP partners, and we aim to build the partnership to at least four partners in the next two years. We have a young multidisciplinary team including salaried GPs, long term locums, ANPs, highly skilled practice nurses, care coordinator, GP assistants, pharmacists, mental health practitioners and social prescribing link workers. The clinical team is supported by an excellent administrative team with experienced managers.
We regularly invest in collective and individual staff training to enable opportunities for professional growth. We ensure team members are included in practice meetings to aid communication. We have a culture where we believe team spirit is strong as is the commitment to great patient care.
The successful applicant will ideally be a Nurse associate who has had experience of the Primary care setting or feels that the training has developed skills that are transferrable to our care setting. We would look for the individual to develop themselves in this role where a service provision development may be required. Primary care clinical experience is desirable in health promotion, communication skills and long-term disease management.
Preceptorship will be offered if the successful applicant has just completed their training.

Salary: £23,000.00-£26,000.00 per year

Lead Nurse Manager

We have an exciting opportunity for a Lead Nurse to join our team at Wensum Valley Medical Practice. We are looking for a dynamic, committed and experienced individual to work collaboratively with the entire practice team, supporting the delivery of care for the practice population, including patients with acute and chronic conditions. The successful applicant will also provide leadership and direction for the nursing team, including Healthcare Assistants and Phlebotomists.
The team at Wensum Valley Medical Practice are friendly and supportive, and we work collaboratively to deliver a high level of care and service for all patients, who currently number around 12,500.
We are friendly and dynamic, and work across three surgeries in Norwich. We have been providing care to the people of Norwich for many years and have a strong focus on patient care. We are passionate about training and supporting our staff.
The successful applicant will be an experienced Nurse Manager, preferably with primary care experience, or someone from a primary care background who is looking to take the next step in their career development. We welcome applications from individuals with solid clinical and management skills who also has a vision for service development and delivery.
Salary to be agreed based on skills and experience.
Job Types: Full-time, Part-time, Permanent
Part-time hours: 30 – 37.5 per week
Salary: £43,000.00-£48,000.00 per year
